Job Summary:

The Care Scheduler is responsible for organizing and coordinating client and Care Professional schedules ensuring timely and efficient scheduling for home health aide services. This role requires strong communication and organizational skills to work closely with clients, Care Pros and support staff, guaranteeing a smooth scheduling process. The ideal candidate will have excellent attention to detail, a high level of empathy, and the ability to prioritize both client and Care Professional’s needs.


Key Responsibilities:

  • Scheduling and Coordination:
    • Schedule and coordinate ongoing home health aide services for clients and corresponding Care Professionals
    • Adjust schedules as needed, considering Care Professionals’ availability and client needs.
    • Confirm schedules and follow up with patients on any changes.
  • Client and Care Professional Communication and Support:
    • Serve as a point of contact for clients, answering questions related to scheduling and ensuring the care needs are met.
    • Serve as point of contact for Care Professionals, successfully matching them with clients that match their skillset and desired hours each week.
  • Record Management:
    • Maintain accurate and up-to-date records of client and Care Professionals schedules in electronic health records (EHR) or scheduling software.
    • Monitor scheduling metrics and help identify opportunities to improve the efficiency of scheduling processes.
  • Collaboration with Client Care Management Team:
    • Communicate with client care management teams of scheduling adjustments or urgent patient needs.
    • Ensure alignment between care schedules and care plans, particularly for clients requiring frequent follow-ups and or specialized care.
  • Problem Solving and Conflict Resolution:
    • Resolve scheduling conflicts by prioritizing urgent needs and collaborating with team members to find suitable alternatives.
    • Troubleshoot any issues that arise with scheduling, such as double bookings or last-minute cancellations and call offs

Qualifications:

  • Strong attention to detail and ability to work in a fast-paced environment.
  • Excellent customer service and interpersonal communication skills.
  • Experience in healthcare scheduling or administrative roles, particularly within a clinical or medical environment.
  • Proficiency with scheduling software, EHR systems, and basic computer applications.
  • High school diploma or equivalent required; associate’s degree or relevant certification preferred.
